OCHC Urbana Medical Center School Based Behavioral Health  counselors provide services to address the behavioral, academic, social, cognitive and emotional needs of our students in order to promote the most beneficial learning environment. Community Health Worker Services

Community Health Workers (CHWs) are public health workers who serve as a liaison between the community and health/social services.  CHWs help patients and their families navigate their health journeys by accessing resources, increasing health education, and helping to adopt healthy behaviors and lifestyles.  CHWs can come to your home or meet in the clinic to discuss your needs and find the resources and services to help.
